About the Role
As an Operations and Enablement Lead dedicated to Care Coordination, you will bridge Care Coordination and SonderMind’s broader technology ecosystem. This role combines tooling strategy with hands-on system expertise: assessing workflows, optimizing processes, and configuring backend systems to meet the demands of a high-volume environment. You will own the implementation and daily configuration of Care Coordination tools, ensuring the team can manage referrals, patient conversion, and partner integrations with efficiency and accuracy.
What you’ll do
- Dedicated Care Coordination Partner: Act as the embedded systems expert for the Care Coordination department, developing a deep understanding of its workflows, tools, and performance drivers.
- Implementation & Project Management: Lead implementation projects for new tools, enhancements, and integrations related to Care Coordination.
- Managed Backend Configuration & Optimization: Partner with BizOps and technical/vendor teams to align system functionality with Care Coordination workflows. Translate operational needs into clear requirements and ensure configurations and automations support efficiency, compliance, and ease of use.
- Cross-Functional Alignment: Partner with technical and vendor teams to gather requirements, test solutions, and provide operational feedback that ensures tools like TalkDesk and Magical effectively support Care Coordination workflows. Facilitate communication between stakeholders to align system updates with frontline needs.
- Support & Troubleshooting: Act as the first line of escalation for Care Coordination system/tool issues, triaging and resolving backend challenges while ensuring minimal disruption to daily operations.
- Data Integrity & Reporting Enablement: Partner with Analytics to ensure Care Coordination data is captured accurately and reliably for scorecards, dashboards, and reporting needs.
What does success look like?
- Within 30 days: You have mapped the Care Coordination tooling ecosystem, met with frontline and leadership stakeholders, and are triaging configuration and support requests with oversight.
- Within 3 months: Partnering on configuration updates and owning operational implementation/documentation
- Within 6 months: You are independently leading small-to-medium implementation projects and are recognized as the go-to resource for Care Coordination tooling and systems expertise.
- Within 1 year: Supporting and executing cross-functional initiatives and surfacing process or tooling improvement opportunities.
Who You Are

4-6 years of experience in business systems analysis, systems configuration, or tooling strategy, ideally within healthcare operations, care coordination, or a similarly high-complexity environment.

Familiarity with workflow automation, system integrations, and vendor collaboration.

Strong technical fluency, ability to understand system logic, workflows, and data flows, and to partner with technical teams or vendors to implement effective solutions.

Demonstrated ability to manage projects, gather requirements, and deliver solutions that balance frontline usability with system integrity.

A deep interest in healthcare workflows and the compliance/privacy considerations unique to the industry.

Bachelor’s degree in Information Systems, Business, or related field (or equivalent work experience).
Our Benefits
The anticipated salary range for this role is $88,000 - $99,000. Final compensation will be determined based on a variety of factors, including relevant experience, skills, education, and past performance. In addition to base salary, this position is eligible to participate in our equity program.
